New client forms will be sent to you through a HIPAA compliant therapy website.  These forms can be completed online once we have arranged for your initial session.  If you prefer to fill the forms out manually, they can be accessed here to be filled out and brought to initial session:

When a client wants to or is willing to authorize a therapist to release the information, the client must complete and sign the Authorization to Release Information:
